AG POLE FITNESS WAIVER T&C Below is our Virtual Fitness waiver for AG Pole Fitness: Please Read, as When you Book a Class your Acknowledged you have Read the Waiver and Agree.
I agree that I am 18 years of age or older.
I do not have any pre-existing health conditions or chronic injuries/illnesses that prevent me from engaging in physical teaching/activities as outlined by AG Pole Fitness.
I understand participation while under the influence of alcohol or any other mind altering chemicals is strictly prohibited. For my safety and the safety of others, I have a zero tolerance policy against participating in classes when under the influence of any substance that alters mental and/or physical state. If I am suspected of being under the influence, I will be required to leave.
I understand engaging in physical activity and attending classes with AG Pole Fitness is done so at my own risk. I assume all risks, dangers, and hazards as well as accept the possibility of personal injury, death, property damage, and loss resulting from engaging in activity at AG Pole Fitness or affiliation.
I am not responsible for personal belongings brought into and/or left in the studio. Your responsible for your belongings.
I agree to maintain the privacy of all members other than myself. Photographs, videos, and/or testimonials are at the explicit discretion. I will not post or share images or videos of other members without their direct consent.
I have read and agree with all terms set forth in this agreement without reservation. I am voluntarily participating in these activities and use these facilities and premises and assume all risk of injury. I discharge AG Pole Fitness and all affiliates from all liability and waive any right to bring legal action against the organization for any and all acts or omissions including but not limited to negligence's, strict liability, breach of warranty and personal injury or property damage or loss.
